Things to remember
Please see below some things to remember when completing your reporting:
If you deliver more than one activity you will need to complete one report per activity.
Please upload all participants registration forms as a PDF document and ensure they are completed in full - the details on the registration forms are vital as they provide all the information that is needed to complete your report/s and also ensures that our records remain as accurate as possible so that exciting activities like this can continue in the future.
If you have any difficulties uploading the registration forms or submitting your reports, please contact me as soon as possible.

All invoices are to be sent directly to Accounts Payable.
After submitting your block reporting, please forward your invoice to einvoicing@logan.qld.gov.au and ensure the following:
•purchase order number is referenced on invoice
•invoice total matches the agreed price of the respective block outlined in 5.0 Payment Schedule of your Active & Healthy service provider contract (not inclusive of GST)
•If you are registered for GST, please add GST to the total cost outlined in your contract.
Your invoice will be processed by a Community Programming Officer once all reporting form/s are submitted for your activity/s.
Please note, if you have already submitted your invoice to Council prior to completing this reporting form, your invoice has not been actioned.
